BY FEDERAL LAW AND AS INDICATED BY THE FUMIGANT LABEL, THE FOLLOWING PREPARATIONS MUST BE COMPLETED PRIOR TO FUMIGATION.

1. REMOVE persons, pets and plants (including seeds and bulbs). LEAVING PETS UNATTENDED IN THE YARD IS NOT SUFFICIENT.

2. REMOVE all food, animal feed and medicinals which are not factory sealed in metal or glass (including freezer and refrigerator items and bottled water). Do not bag anything other than edible foodstuffs or medicinals. As an alternative to removal of these items, they may be sealed by double bagging in Nylofume bags. These protective bags are available from Key Termite and Pest Control. 10 bags (5 finished bags) and bagging instructions will be provided upon request at no extra charge. No other bags are legally allowed. If food has not been removed or properly bagged at the time the fumigators arrive, there will be a minimum $100.00 additional charge for our crew to bag the foodstuffs or to re-bag improperly bagged foodstuffs. There is also the possibility that these items may bediscarded. Key Termite and Pest Control is not liable for replacement costs of any spoiled, discarded or overlooked items. Any undiscovered edibles found after the fumigation must be discarded.

3. Climbing plants and vines and/or trellises must be detached prior to the arrival of the fumigation crew. Toallow for ground seal and to reduce damage to the plants in close proximity to the house, have all tress, plants andfoliage cut back at least 8-12 inches from the building’s walls and roof. Plants left unattended which are too close tothe building to allow for sandbagging of tarps will die or be damaged in the fumigation. Key Termite and PestControl will assume no responsibility for plant damage. To minimize escape of the fumigant through the soil,remove all ground cover (bark, gravel, decorative rock, etc.), remove storage away from building perimeter and wetthe soil adjacent to the structure prior to the fumigation. Additional charges of $45.00 per man hour will be billedto the homeowner for plant trimming or ground preparations completed by Key Termite and Pest Control.

4. Mattresses that are completely covered with waterproof covers (except waterbeds) or pillows completelyenveloped in waterproof covers must have covers removed. If plastic covers are removed, mattresses andpillows may remain. BABY MATTRESSES MUST BE REMOVED WITHOUT EXCEPTION.

5. Electricity and water must be on or available in order for the fumigation to take place. NOTE: In areas servicedby PG&E for their gas service, they currently require that only PG&E employees may disconnect and reconnect gasservice. Check with your termite inspector as to the procedures to be followed in these locations. In other areas, wewill turn the gas off prior to the fumigation and upon completion of the fumigation’s, we will turn the gas back onand relight the pilot lights. Key Termite and Pest Control will assume no responsibility for malfunction of gasappliances when relighting pilot lights. In such instances, a professional should be contacted at the owner’sexpense. If you have more than one antenna or antennas over 12 feet high, as well as CB and short wave antennasand satellite dishes, please make prior arrangements for their removal and re-installation. If any part of your antennais in poor condition, such as mounting brackets, guy wires, etc., the antenna will not be reinstalled.

6. Fumigation will be completed by industry standards and reasonable caution used, but in certain situations, somedamage may occur to roofing (i.e. old brittle shingles, metal, slate, or tile), awnings, vent caps, spark arrestors, solarpanels, patio covers and certain rain gutters. Paint may peel where tape and adhesive spray are applied to areaswhere the pre-painted surface was not adequately primed or prepared prior to paint. Key Termite and Pest Controlassumes not responsibility for damages to painted surfaces should they occur.

7. All persons should be prepared to vacate the premises when the fumigation crew arrives. We will not attemptto work around moving crews. All rooms must be left unlocked, including padlocks on garage doors or other storageareas. Pet doors must be locked or secured against entry. Locked cars may not be left in garages or carports. If carsmust remain, doors and trunk must be left open. State law requires that we have a signed Occupant’s FumigationNotice in our possession, as well as the keys to the structure, prior to the beginning of the fumigation. Please notethat the fumigation cannot be performed during inclement weather conditions. In case of rain or strong winds, thefumigation may be postponed to a later date. Flat roofs (especially metal carport and patio roofs) are vulnerable tothe accumulation of water on top of the tarps and may collapse. Water could collect quickly and weigh hundreds ofpounds. Key Termite and Pest Control assumes no liability for such damages should they unexpectedly occur.

8. If the fumigation must be canceled because of inadequate preparation of the structure, a cancellation charge of10% if the fumigation cost may be applied. A cancellation charge may be applied if the fumigation cannot beperformed, due to lack of preparations on the part of the tenant, owner or agent. These reasons include, but are notlimited to: keys unavailable to all locked rooms, safes or spaces; locked cars in garages; storage with unknowncontents which may include foodstuffs; improper preparation of plants, rocks or landscape materials; building underpartial construction, especially roofs in an unsafe condition due to construction. All alarms must be disarmed duringthe fumigation as well as motion detector lights that will be under tarps or nearby which could cause a hazard. Inaddition, all automatic-watering systems must be made inoperable. Drip irrigation that is against the house or is lessthat 12 inches from the structure should be disconnected or may become damaged. In the event that a locked gate,which is used as security to prevent entry to a swimming pool, must be rendered unlockable, another means ofsecurity must be provided.

9. Swallows often nest in the eave areas of buildings. This usually occurs in the spring and early summer.However, if the nests are occupied, federal law prohibits us from disturbing them, whether by fumigation orremoval. Under no circumstances can we proceed until the nests have been vacated.

***Do you have any knowledge of any apparatus or equipment located on the ground, underground oroverhead which might allow the fumigant to travel from the building(s) being fumigated to any adjacent occupiedstructure? These means of connections may include, but not be limited to, underground plumbing lines which arenot sealed with water, overhead or underground conduits which are connecting one building to another orconnecting central vacuum systems. Please advise us if you know of these or similar connections.

***Key Termite and Pest Control does not provide on-site security and does not assume any responsibility for thecare and custody of the property in the event of vandalism or breaking and entering. The company suggests that allvaluables such as jewelry, coins, collections, cash, art objects, guns and other items of value be removed from thesite, prior to fumigation. If you wish, you may hire a security service to be present during the fumigation.
